I am an out-of-network provider for several insurance companies. To determine if you have out-of-network coverage, please contact your insurance company.  If you receive out of network benefits with your insurance coverage,  upon request and following your payment of my private practice rate, I will provide you with a "Superbill".  A Superbill contains all of the necessary information for you to submit a claim to your  insurance company for reimbursement of services.  They will reimburse you per your member agreement with them.  Please note, I do not guarantee that your insurance company will reimburse you for services and you should always contact your insurance provider in advance to determine your coverage.

A card is required to be on file to reserve your appointment and full payment is due  prior to the session.  I do not have access to your card information, as the storage and processing of financial information adhere to Payment Card Industry Data Security Standards (PCI-DSS) for protecting cardholder data. I accept payments via debit/credit card, and HSA.

Cancellation Policy

If session is canceled or rescheduled more than 24 hours in advanced, you will not incur any fees.  If you need to cancel or reschedule an appointment, you must do so prior to 24 hours prior to the scheduled appointment to avoid being liable for any fees.   If session is canceled or rescheduled with less than 24 hours notice, 50% of the rate is charged.

Good Faith Estimate

Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to provide a good faith estimate of expected charges for items and services to individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage, both orally and in writing, upon request or at the time of scheduling health care items and services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services. You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service, or at any time during treatment. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.

For questions or more information about your right to a Good Faith Estimate, or how to dispute a bill, see your Estimate, or visit www.cms.gov/nosurprises.
